Supervises the daily clinical operations of the RN Case managers in both the University Hospital and Cardiovascular Center and partner/support MOTT/children's supervisor. This includes coordination of care and discharge/transition planning functions to facilitate the placement of patients in the appropriate level of care across the continuum, or seeking alternatives to admissions.
Actively participates in the design, implementation and evaluation of practices and processes which meet and exceed regulatory agencies' requirements and evidence-based standards of performance.
Ensures that clinical staff meets these requirements and standards through competency-based training during orientation, annual, on-going continuing education and performance improvement plans. Focuses on the development of high performance teams to collaborate with multidisciplinary members of the inpatient unit or emergency departments.
Ensures accurate and timely communications/handoffs for patient safety at transition to next level of care.
Actively participates in the identification, implementation and evaluation of individual and department performance improvement opportunities.
Responsible for providing leadership and professional expertise to develop and leverage the knowledge and skills of others. These skills/behaviors include: critical thinking/complex problem-solving to resolve barriers to discharge and other issues; coaching and mentoring staff to achieve higher levels of performance; responsiveness and sensitivity to internal and external patient or customer concerns.
To achieve hospital throughput and other department/organization goals, collaboration with all levels of staff and disciplines is required.
Reports to the Associate Director of Care Management. Provides functional and administrative support of nursing and support staff as delegated by the Associate Director.
The duties of the Supervisor, Clinical Nursing are:
- Participates in the interview, selection, training, and on-going continuing education of RN Case Managers and support staff.
- Plans and assigns case managers to specific staffing and schedules, upon the needs of the patient populations and budgetary requirements; makes readjustments based upon acuity and volume.
- Monitors and evaluates Case manager work and productivity; coaches for best of class performance
- Ensures Case Manager accountability and compliance with mandatories, and all department competencies.
- Works to resolves employee issues or concerns in accordance with regulatory agencies requirements, standards of performance and UMPNC Bargaining Agreement.
- Constructs and/or participates in individual employee performance improvement plans
- Provides progressive disciplinary action for attendance and performance in accordance with UMHS policies and procedures and UMPNC bargaining agreement.
- Applies and communicates principles of case/utilization management for discharge planning, such as: clinical criteria, insurance, alternate level of care placement, negotiation of benefits, length of stay, readmission rates or others
- Resolves internal and external patient or customer concerns; performs service recovery as needed
- Assists with review and on-going revisions or updates to department Policy, Procedure and Processes Manual
-Assesses overall department performance on a regular basis; recommends, implements and evaluates,
opportunities to achieve performance metrics
-Assumes responsibility for completion of assigned special projects and their outcomes.
- Implements all UMHS policies, procedures, and quality and safety programs.
- Participates in workgroups, committees and planning activities for department and organization.
-Serves as a role model to all staff members through demonstration of Case Manager competencies
Educational Requirement:
Bachelor's degree in Nursing OR Master's degree in Nursing
Current licensure as a Registered Nurse in the State of Michigan.
Minimum 3 years of recent (within the last 5 years) RN experience in Discharge Planning, Home Care, Case/Utilization Management or relevant experiences required
Computer and word-processing skills with knowledge and application of multiple software programs (Word, Excel, Power Point, Access, etc.)
Demonstrated ability to perform RN Case manager competencies/behaviors
Masters degree in Nursing preferred.
Demonstrated excellence in verbal, non-verbal and written communications
Data-driven decisions used to resolve conflict
Ability to manage multiple competing priorities in a timely and accurate manner
Demonstrated excellence in problem solving and interpersonal skills.
Knowledge of Michigan Medicine policy and procedures
Familiarity with UMPNC bargaining agreement
